The purpose of this form is to document preliminary information to determine whether an investigation is warranted regarding improper or neglectful business practices within the United Application Standards Group.

The purpose of this form is to document preliminary information to determine whether an investigation is warranted regarding improper or neglectful business practices within the United Application Standards Group. This info will be sent to the Ethics Committee Chairman. The information will be reviewed and a determination as to whether to proceed or not will be made. This information will be kept confidential until a cause to act has been established. If it is determined that a further investigation is necessary, this will be presented to the entire Board of Directors for review. The name of the complainant will still be kept confidential. If action is warranted, all parties involved will be notified. The Board will complete the investigation and notify all parties of the final conclusion. If the complaint is against the Ethics Committee Chairperson, the president and one other board member will determine the validity of the complaint and normal procedures will apply.
