Membership Renewal

You must send a current paystub for your tested installer at the time of renewal. Documents should be attached in the field below or faxed to Sharon Paxton (714-281-2206).

The installer that tested is still employed full time at my company. My company has not filed bankruptcy, either Chapter 7 or Chapter 11.

• Phones are answered promptly and in a professional manner.
• The office and/or entry area is well organized.
• Employees dress in a professional manner.
• Files and job records are organized.
• Specific job records can be located quickly.
• Job records contain detailed information, including vehicle unit numbers, serial numbers, site location, temperature, and other miscellaneous information.
• Photos are regularly taken of our projects.
• Photos are retained and kept in an organized fashion.
• Non-warranted installations are noted on job records.
• Employees are educated on proper installation techniques.
• Personnel files are maintained in accordance with state laws.
• Federal and state employment posters are placed where employees can easily access and/or view.
• Material safety data sheets are organized and available to employees.
• Equipment and tools are maintained in a safe working order.
• Safety meetings are held regularly with staff.
• Employees have access to 3M Product Bulletins and Technical Information.

By agreeing below, you are acknowledging and certifying that you are upholding the standards of the organization and that all of the above information is correct and true to the best of your knowledge. You also understand that if any of the above information is found to be false or grossly inaccurate, your membership and/or certification may be revoked without a refund of fees or dues.

Code of Conduct

1. Members will operate their business with honesty and fairness. This pertains to employees, customers and other members.
2. Members will not back-solicit other member’s customers.
3. Members will strive to achieve and maintain the highest level of professionalism and application standards within their own companies.
4. Members will promote a friendly and harmonious atmosphere at all association functions and meetings.
5. Members will not degrade, criticize or harm another member’s person or reputation.
6. Members will strive to protect the welfare and reputation of other members and the UASG.
7. Members will warn and protect other members and the association when issues arise that could cause financial hardship.
8. Members will not solicit potential employees by initiating contact with UASG member employees concerning potential employment opportunities with own company.
9. Members will not use confidential and proprietary UASG member company information for personal gain without prior consent.
10. In regard to sub-contracted labor, members will only represent work as being a certified graphics installation when performed by an employee of a UASG Certified company.
